Referring a patient for admission
Patients are typically referred to a Select Specialty Hospital by their physicians or discharge planners (social workers, case managers) at the acute care hospital or other health care facility where they are being treated.
Our Select Specialty Hospital clinical liaisons help coordinate your transfer. If possible, they will meet with you and your family to discuss admission, answer any questions and help expedite the review process.
