MyChart

MyChart is a secure platform providing online access to your medical records from our hospital, part of the Select Medical health network. This platform is designed to help manage your health care. It allows you easy access to information about your medical history and treatments and gives you the ability to download copies of your records.

To set up MyChart, patients will need a personal activation code. There are two options for obtaining personal activation codes:

An after-visit summary will be provided to patients upon discharge. The summary includes a personal activation code. That code is active for 14 days post discharge.
If your activation code expires or if you need a new activation code, email [email protected] or contact our Health Information Management (HIM) team at (717) 409-7688.

Once you receive the code, click on the MyChart button below to complete set up or access your MyChart account.

You’re ready to go. Log in to the MyChart app, choose our organization and select View Hospital Stay.

Medical Records

Patients occasionally need copies of their medical records for other health care providers or personal reasons. Patients can also authorize a personal representative (family member or caregiver) to receive their records.

To obtain your records, please submit a completed Patient Access form to our Health Information staff.

You will be notified of any fees, if applicable, before records are released.
