Select Specialty Hospitals have developed a way to communicate with you that is easy to understand and confidential: a personal email update.

Our Interdisciplinary Team meets to discuss your family member’s condition and focus on solutions to their medical problems. They also discuss the length of the patient's anticipated stay at Select Specialty Hospital. This is the information that can be shared with you through our CareNotes program.

CareNotes is not intended to substitute for your dialog with the physicians. Plus, you may be requested to participate in family conference(s) to discuss medical and planning issues. Be sure to respond to any requests to meet with the medical team or the Case Manager. These meetings are an important part of your family member's treatment plan.

For security and privacy, Select disseminates patient information through a secure messaging service called Zixcorp®.

Using CareNotes is simple

1. Complete Patient Authorization form
2. Receive first email. Establish confidential password.
3. Open Attachment 'CareNote_Update.htm'
4. Save your CareNote to a document for future reference
