Patient Referrals

Patients are referred to a Select Specialty Hospital by a variety of sources: physicians, social workers, case managers, Nursing Homes, insurance case managers, Home Health Agencies, patients and or families or caregivers.

To refer a patient to a Select Specialty Hospital, call the hospital's main phone number, or complete a Patient Referral Form. When calling a Select Specialty Hospital, ask for the Admissions department, which will take the referral information and complete the intake process with you.
